You may need to create an action plan to record agreed improvements, outline required follow‑up actions, and track progress with the provider after an assessment.
Any questions that received a rating of Requires Improvement or Poor in your assessment, need to be addressed. This is achieved by the provider creating a Draft Action Plan, which is then reviewed by the assessor.
To create a draft action plan, follow these simple steps:
1. Go to the provider portal and open your assessment.
2. Click Areas For Improvement.
3. Click Create Draft Action Plan.
4. Click Add Action to add an action to the plan.
5. You can add additional actions by returning to the Action Plan tab and selecting New Action.
6. You can also modify existing actions by selecting the Edit icon and then selecting Modify Action.
7. Once all actions have been added, you can click Submit Draft Action Plan, which will then go to the assessor to review and approve.
🤓 Tip: Once you have submitted, the status will show as Awaiting Approval. You are not able to make further changes whilst this status is displayed.
